Each threshold is worth exactly four right answers in its section. Because every wrong answer cancels a quarter of a right one, you reach a threshold when your right answers, less a quarter of your wrong ones, come to four or more.

A score below the threshold in any one section keeps you out of the ranking, whatever your total. There is no separate total threshold: the 192 shown in the official structure table is the sum of the three section thresholds.
Two rankings and the tie-breaks.
Humanitas draws up one ranking for Medicine & Surgery and one for MEDTEC. If you apply for both, you appear in both, each time with the score that programme’s weights give your answers. Your best score across the two rounds of the year counts.
